Membership Application:

 

If you would like to join Vista Art Foundation, please fill out and print the Membership Application Form

 

This is a writable PDF form.  Fill it out and then print it immediately by clicking the print button in the upper left corner.  You can not save the form with your information on it.

 

Artists who wish to display art at the gallery also need to print and sign the Member Artist Waiver Agreement.  Return both documents together to VAF. 

 

Monthly Exhibit Entry Forms:

 

You have a choice of two options:

 

Option #1:  Click here to bring up the forms in writable PDF format.  Type in your info, and then print it immediately by clicking the "Print Form" button in the upper right corner.  If that does not work, click on the printer icon on the left.  You can not save the form with your information on it.

 

Option #2:  Click here to bring up the forms in Word Document form.  Save the Document to your computer (someplace you will be able to find it whenever you want it, with a name you will recognize, such as "VAF Intake Forms").  Then you will be able to fill in your name and phone in all four quadrants and save that.  Then fill out as much as you can and print it however you usually print Word documents.  One caution:  make sure you get your cursor positioned just above the line where you want to type.  If you mess up the form, click on the "undo" arrow (or click on "Edit" then "Undo" and keep doing that until the form is back to the way it was; or, simply close it without saving and open the saved form and start over.

 

 

 

Instructions for Wiring Artwork

 

  One day Bob Boyd was hanging art at VAF when he heard a noise and realized that a piece he had hung moments earlier was now on the floor.  This crash was due to improper wiring.  Bob has written instructions for secure wiring of your artwork (including photographs of each step).  Joe Libertini contributed a helpful chart on the last page.
